Before You Schedule

Please Read All Information Carefully Before Scheduling an Appointment

When scheduling please include:

1. Active insurance, Member ID, and Group Number in the insurance box

2. Reason for visit

Please email the front and back of your insurance card to team@intricatehealth.net. This will aide in correct billing the day of the appointment and ensure that we are in network

Do NOT schedule lab-work unless you have already been seen and labs were ordered

Do not schedule Aesthetic procedures online. We will be more than happy to assist you as we have dedicated appointments

There is a No Show Fee of $40
