Before You Schedule

Please Read All Information Carefully Before Scheduling an Appointment

When scheduling please include:


1. Active insurance, Member ID, and Group Number in the insurance box


2. Reason for visit


  • Please email the front and back of your insurance card to team@intricatehealth.net.  This will aide in correct billing the day of the appointment and ensure that we are in network


  • Do NOT schedule lab-work unless you have already been seen and labs were ordered


  • Do not schedule Aesthetic procedures online.  We will be more than happy to assist you as we have dedicated appointments


  • Feel free to call our office with questions prior to scheduling 469-640-6423

Cancellation & No-Show Policy

 A $40 fee will be assessed for appointments canceled or rescheduled with less than 24 hours' notice, as well as missed appointments (no-shows).


To avoid this fee, please cancel or reschedule your appointment at least 24 hours before your scheduled appointment time.


Important: If our online scheduling system no longer allows you to cancel your appointment, you are within the cancellation cutoff, and the $40 fee will apply.
